She got to the hospital at 4cm and the doctor popped her water. She had abnormally strong contractions and dilated from 4 cm to 10cm and pushed him out within LESS then 2 hours after showing up to the hospital. The doctors didn't have enough time to give her any anesthesia. Her IV didn't even have time to finish..... The contractions were so strong that baby was actually coming out of the birth canal and the babies head was completely out of her vagina before she even started pushing!

This video shows an ECV to turn a breech baby to head-first. It is offered to women at 36-37 weeks and reduces the chance of needing caesarean section. It is a safe procedure and is successful about 40-50% of the time. On this occasion I used a drug to help relax the womb first. While most women find it uncomfortable, it isn't usually painful. If you have a tender uterus anyway, it is more likely to hurt.

The baby in this video had a reassuring fetal monitoring right before I started the procedure and the fetal heart was checked afterwards. If a longer attempt at ECV was carried out then we check half way through too with the ultrasound. Transient changes in the baby's heart rate are not uncommon and usually settle after a few minutes. Very occasionally they don't settle and a caesarean section needs to be done.

For the ECV shown, the baby does a forward somersault anticlockwise. The first step is to lift the baby's bottom out from your pelvis and move her to lying transversely. Often that is the most difficult part. Overall, it normally takes between 1 and 2 minutes to do the turn - any longer than that and the chance of success goes down. It is more likely to succeed in the following situations:

- if you have had a baby before
- if there is a normal amount of water around the baby
- if your baby has his knees bent
- if his or her bottom isn't engaged in your pelvis
- if the baby's head is not right up against the placenta

During an ECV uterine contractions can occur and we generally just wait until they pass.

Is it safe?

One of the best quality research on safety comes from Oxford in the UK (incidentally, where I trained to do them) and they looked at just over 800 consecutive ECVs. The chance of needing emergency CS during or just after the ECV was 1 in 200. The risk of the placenta coming away (abruption) was 0.1% and no babies died due to the procedure. These figures are similar to other research that is available and confirm the safety of ECV. Approximately 3% of babies turn back to breech after successful ECV and 3% of unsuccessful ECVs turn spontaneously to head-first afterwards.

This was one amazing delivery I performed. This woman was NOT in labor, but delivered 5 children before and had a pathologically loose birth canal (had to use pessary during the pregnancy to prevent prolapse). At around 39 weeks she came in to be checked. She was like 4cm dialated already. Almost as a joke I told her to give me one push. That put her cervix to 8cm. I took her to the hospital and told her that we will try something unusual, to deliver this baby without labor. She pushed that baby out in 10 minutes. Amazing.

i'm the baby's mother....They put the tube in to suck out the fluids in the baby's breathing passage... but i'm not sure why they are not using the small balloon type of suction..but is fine it just a tiny and soft tube it doesn't hurt the baby.I was a nurse in Assunta Hosp.. honestly , he is a very nice doctor and the nurses are helpful and friendly !! You choose the right place ...welcome to the motherhood !!! :)

Sometimes what's best for mom - is not best for her baby. Hospitals nationwide are seeing more pregnant women choosing to deliver early, in order to accommodate their busy schedules. But that decision comes with huge health risks for their babies.

What a difference a week makes.

"A baby's brain at 35 weeks is only two-thirds the size it is at 40 weeks. That's a huge amount of growth that baby does," said Dr. Mary Yankaskas, OB Medical Director at Lee Memorial Hospital.

Doctors at Lee Memorial say they've seen more women choose to induce birth or have C-sections at fewer than 39 weeks - thinking it's safe, when it's really not.

"They want to plan the exact day their child is born. They want to plan who their physician is. They want to plan having their family come in. They seem to plan their whole life around the delivery of their baby," said Chris Jabaay, Director of Labor and Delivery at Lee Memorial.

But research shows what's more convenient for moms has huge health risks for their babies.

Last year, Lee Memorial had just as many 35 week babies in their Neo-Natal Intensive Care Unit as 38 week babies.

"Breathing problems, temperature problems, feeding problems, bonding problems," said Jabaay.

So the hospital has partnered with March of Dimes to launch a new initiative - completely eliminating elective deliveries before 39 completed weeks of pregnancy.

"When they learn that their baby's brain is not totally developed at 37 weeks, they are not wanting to deliver at all. They want to wait 39 or more," said Jabaay.

This would not apply to women who have medical issues and need to deliver early - only to women choosing to deliver early. Lee Memorial expects fewer admissions to their Neo-Natal Intensive Care Unit and healthier babies.

"Changing our policies and our practices - doing what's best for the patients and for the baby," said Dr. Yankaskas.

Lee Memorial is one of six hospitals in Florida - the only one in Southwest Florida - that's part of this 39 week initiative. It has also been launched in New York, California, Texas and Illinois.

Five Reasons To Stay Home In Early labor

In the movies, the water breaks, everyone flips out and the next thing you know mom’s being raced down a hospital corridor on a gurney. But in real life, especially for a first labor, this is very, very rarely the case. Still, expecting women and partners are often shocked to learn that they will be sent home if they “rush” into the hospital at the first contraction or even when the water breaks.

In the absence of any particular concerns—like a preexisting maternal health condition that might require close monitoring—women can do a lot of laboring at home.

The idea of mom being in actual real labor at home, unsupervised by medical personnel, can be a bit disconcerting to those of us raised with The Emergency Birth narrative pummeled into our heads. Partners in particular can feel a bit out of their depth and eager to get a professional on board. I completely understand that feeling.

But there are logical, evidence-based reasons for staying home until active labor has really kicked in.

Before I list them, a quick breakdown of “early” vs “active” labor: Early labor, which can last hours or days, is when the cervix dilates the first three centimeters– contractions can be anywhere from 5 to 30 minutes apart and less than a minute long. These contractions can be mild to difficult– they may be strong enough to require some pain-coping, or easy enough to mostly ignore.

Active labor starts when those contractions move to about 3-5 minutes apart and last about one minute a piece. This is the more intense part of labor when pain-coping techniques need to be put to use and the mother needs more focus. It can still last several to many hours for first time mothers; this is when she dilates the next 7 centimeters. So leaving for the hospital sometime in early to mid-active labor will still get you in to the hospital with plenty of time barring serious commuting distances, in which case talk to your care providers about the best protocol. Now onto the reasons for staying home:

Staying home in Early labor….

1. ….reduces the chances of induction and c-section.

At some point it was decided that a woman should dilate about one centimeter per hour. If this were true, most women would labor for precisely ten hours and bam: baby in arms. But that doesn’t happen.

This expectation—based on averages—has led to an increased use of induction drugs to move along supposedly slow labors. But are they really slow? A first labor can be ten hours, but it can be longer, too. Or shorter. Also labors have a normal ebb and flow to them. They can slow down and speed up for any number of reasons.

The averages are useless to almost all of us. And yet they are used to determine how we are “progressing.” If you come in to the hospital in early labor there’s a chance you’ll be declared “failing to progress” and given induction drugs that, while very useful in some circumstances, are not without risks. Induced labors are twice as likely to result in a c-section.

2. …. helps labor progress.

The labor hormone oxytocin requires a little hand-holding to get going. Oxytocin is an incredible hormone, it’s what causes contractions but it’s also responsible for feelings of love and bonding. It’s also released during breastfeeding, orgasm, falling in love, when you give, when you get a massage, when you feel safe and when it’s dark.

Oxytocin is not a fan of adrenalin and logistical maneuverings. It likes intimacy, basically.

This is one reason so many labors start at night. OK, so given this physiological set-up, why “rush” to a fluorescently-lit hospital? Oxytocin will flow a lot faster if you’re at home, in bed, in the dark or in the bath or with someone you love. Once that hormone is really flowing, it’s hard to shut down. But early in labor, it’s a bit more fickle. So give it some nice cozy environment to get that show on the road. Get it ramped up to the point of no return, so you’re labor will be faster and more efficient once you do get to the hospital. You may even meet those one centimeter an hour standards, if you’ve let that oxytocin build up a whole lot.

3 …means you can eat and drink, which is actually good for you in labor.

Hospitals are lagging behind the research in this area. Some places are allowing food and drink in labor wards, but many still offer IVs or ice-chips. Women, especially first time mothers who may be laboring for about a day (or night) can use some sustenance. This is muscular work, after-all. Most women don’t want to eat during very hard active labor—sips of water or Gatorade usually do the trick. But in early labor you can snack all you want.

4 …..gives you more flexibility about pain-coping techniques.

Some hospitals have birthing tubs for labor, but many do not. And water has been shown as effective in reducing pain in labor as a shot of narcotics. Taking a nice long bath during painful contractions can be very relaxing—mentally and physically.

You may also be able to move around more, which is so helpful. Women are monitored for some stretch of time upon entering a birthing facility. This requires being on your back and strapped to various machines. Not so comfortable and not as efficient for labor—changing position is very useful for taking the edge of pain but also for *helping labor progress.” I encourage you to move around the hospital and try different positions on top of and alongside the hospital bed but it’s nice to get some of those moves well established at home first.

5. … means you’re less likely to be sent home.

Women do get sent home if they come in too soon. Some midwives and doctors will want to listen to you have a contraction over the phone. If you are able to talk through it, it’s probably not active labor and probably not time to come in.

The bottom line is that your labor will seem, and possibly even be, shorter if you stay home for the first stretch—this can be hours or even days if it’s a very long early labor. (Some labors take a long time to ramp up.) You can and should call your care-provider once you think labor has started, but you don’t necessarily have to “rush” to the hospital.

So what can you do during all this time? Ignore the labor for as long as possible is usually a good idea. Distract yourself or sleep if you can. Have your partner massage you, watch movies, eat good food, relax, go for a walk, stretch out on the couch with a good book. Home birth fall 'disappointing'

The National Childbirth Trust and the Royal College of Midwives said the drop in the home birth rate, from 2.9% in 2008, to 2.7% in 2009, was "disappointing".

The figures also show an increase in multiple births.

This was most marked in women over 45, where around one in ten had twins, triplets, quads or more.

Recent government policy has been to give women choice over where to give birth - whether in hospital, at home or in a birthing centre run by midwives.

It followed a dramatic fall in births at home in the UK in the last 30 years.

In the 60s, around one in three women gave birth at home. This fell to a record low of one in a hundred home births by the late eighties.

The proportion of home births has risen slightly every year since then, with a small decrease between 2008 and 2009, according to the figures from the Office for National Statistics (ONS).
Choice

In England, 17,834 women (2.7%) had a home birth in 2009, down from 2.8% of home births in 2008.

Wales fared better, with 3.8% of 34,574 births at home, an increase over the previous year.

In Scotland, 873 women (1.5%) had a home birth, out of 59,363 births. And in Northern Ireland, 91 women (0.4%) had home births out of a total of around 25,000 births.

Cathy Warwick, General Secretary of the Royal College of Midwives, said the drop in the home birth rate was "a real disappointment".

She said: "These figures suggest to me that we are not providing the choice that women want and deserve, and that commissioners are not doing enough to offer them that choice.

"My worry is that increasing pressures and demands being made on midwives and maternity services are driving out choice for women.

"There is a real need to look behind these figures to find out why our home birth rate is so low and why it is falling."

Mary Newburn, of the charity The National Childbirth Trust, said they believed women were finding it more difficult to book a home birth.

She said: "There is no evidence of a reduction in demand, but we know maternity services are additionally stretched due to a rising birth rate and too few midwives.

"The option of booking a home birth should be offered as a mainstream option for all women who want it, alongside options to book for care at a birth centre and at a hospital maternity unit."

The ONS figures show that overall, births fell in 2009, the first annual decrease since 2001.

I really enjoyed watching these videos while I was pregnant, so I figured I would give other pregnant women another video to view. I was on the max amount of pitocin when the doctor broke my water. She warned me that my contractions would get stronger, but I had no idea how instant it would be. By far the worst pain I could ever imagine. I suffered through the pain for about 45 minutes before I got my epidural. Getting an epidural was the best choice for me and I definitely recommend getting one. I was smiling through the actual birth and my push time was only 15 minutes. She was born 7 lbs 3 oz and couldn't be happier or healthier to this day. Thanks for watching and enjoy!

Jackie and Craig Winkler tried for 10 years to get pregnant.

In the meantime, they built their careers and social lives. She's in marketing at John Deere and teaches Jazzercise several times a week. He's just weeks away from becoming a licensed practical nurse and is busy with coursework and clinicals.

Like many couples, their lives are hectic right now and while they would have made it work, making room for a traditional weekly prenatal course before their baby's March 16 due date would have been tough.

The parents-to-be were happy to learn they could still get the prenatal knowledge they needed without actually attending the course. Covenant Medical Center recently started offering an online childbirth education class that covers everything from the anatomy of a pregnancy to breast-feeding your new bundle of joy.

"This is a great option for those who are too busy to take the class or who prefer to have the information at their fingertips to access at their convenience," said Teresa Evans, the hospital's director of women and children services. "The upcoming generation of mothers are more technologically oriented and like to do their research online."

Evans said the program also is perfect for mothers who have been ordered to bed rest by their doctors.

Each participant receives access to the online program for six weeks, a "Gift of Motherhood" book and a tour of the birthing facility. Jackie Winkler likes the convenience of learning more about the birthing process on her own schedule, but said the option to go at her own pace - skim through the concepts she already understands and reread those she doesn't - has been invaluable.

Craig Winkler has viewed live births during his coursework. But he has yet to partner someone through the process.

"I know more than the average Joe, but I have been going through some of this with her to learn how I can help," he said.

The program offers an audio/video option, with an instructor reading through the course work, as well as videos and pop-up notations with extra information. Tests at the end of each chapter ensure that participants actually understand what they are reading and seeing.

The hospital still offers the traditional classroom option for prenatal basics, Lamaze and breast-feeding. Evans recommends those who use the online option sign up for the course during the last four months of their pregnancy so they also have access to the program during those first few weeks of motherhood.

"When a new mom is home alone at 3 a.m. and doesn't know what to do with the baby, this is a good, broad overview of all the basic topics," she said.

McHENRY – Children lost to miscarriage, stillbirth or another manner during the time surrounding childbirth will be memorialized during Centegra Health System’s annual candlelight service at 7 p.m. Dec. 1.

The service takes place at Church of Holy Apostles, 5211 Bull Valley Road, McHenry.

Table space will be provided for those who would like to share memories of their child. A child’s name also may be printed in a memorial bulletin for the service. After discharge from the hospital, the support remains available at no charge through individual and couples support and a perinatal grief support group, according to a news release from Centegra.

What to tell the kids

Children don’t develop in chronologically predictable ways. The best way to know when they are ready to understand this kind of information is to listen to what they are asking you.

Always answer children’s questions as honestly as you can — and shut up when they lose interest. Children process only the information they are capable of processing.

Let your child know the difference between your opinion and fact. So you could say that, yes, kids as young as 16 can have a baby, but that you don’t believe they are ready.

From elective caesareans to celebrity birthing gurus, childbirth in the Noughties was all about options. As the NHS slipped further into its maternity crisis, we realised that if we wanted a good birth, we were going to have to sort it out ourselves.

It helped that birthing became cool. Glamorous celebrities such as Cate Blanchett and Victoria Beckham gushed about their blissful waterbirths or their luxurious caesareans. And we wanted what they were having. This, it turned out, was a birth guru.

This expert could perform candlelit caesareans or offer ayurvedic herbs alongside your epidural. When supermodel Elle Macpherson and actress Gwyneth Paltrow endorsed holistic obstetrician Gowri Motha, women threw themselves into prenatal regimes with reiki, reflexology and self-hypnosis. Other A-listers, such as Kate Winslet and Blanchett, turned to midwife Zita West for nutritional supplements and acupuncture alongside their pelvic exams.

Hypnobirthing (self-hypnosis for labour pain) also took off. Initially, this was seen as a bit barmy. But thanks to the birth gurus and positive magazine articles, along with actresses such as Jessica Alba, who likened giving birth to "meditation", we opened our minds. By the end of the decade "hypnatal" was hip.

Other "fringe" ideas became more widely available. At the start of the decade, if you wanted to labour in a water pool you probably had to hire one yourself – and haul it to hospital with you. Now, however, most maternity units in Britain provide birth-pool facilities. Both the Royal College of Obstetricians and the Royal College of Midwives endorse water birth, where appropriate. Clinical studies – and the popular vote – won out.

Of course, medical interventions continued to rise, too. The caesarean rate soared to over a quarter of all births, and more women than ever opted to go under the knife, even if there was no medical reason to do so. For a while it seemed that we were all in danger of becoming "too posh to push".

But, in fact, many of us were desperate to avoid the operating theatre. And with overstretched midwives unable to offer one-to-one support, we increasingly looked outside the system. There we found the doula. A doula is a childbirth companion, trained to offer emotional and practical (but not medical) support during and after birth. Reliable studies show that doulas mean fewer medical interventions.

"Five years ago, nobody had heard of us," says doula Valerie Goodkoep. "Now everyone wants a doula." In 2004, about 700 women hired one via the umbrella body Doula UK. By 2008 this had risen to 2,500. Doulas also worked hard to become available to those who couldn't pay. It is now possible to find a doula, pro bono, via Sure Start schemes or hardship funds.

Above all, childbirth was demystified in the Noughties thanks to the internet. Pregnancy chat rooms and baby websites meant that no discussion, however gory, was out of bounds. Ten years ago, a labouring woman would probably never have witnessed a real birth. Nowadays, she'll have seen hundreds – via video clips that people post on YouTube every day. So, gurus beware: we're all experts now.
